The challenge
Providers were spending more than three hours a day on documentation, and the drag showed: provider satisfaction sat at 56% before the program. Risk capture was suffering with it, with HCC coding performance leaving complexity, and the revenue tied to it, undocumented.

What Scribe-X put in place
US-based remote medical scribes took over real-time documentation directly in the EHR, integrated into existing workflows. Because the scribes carried coding-relevant detail into every note, HCC capture improved as a byproduct of better documentation rather than a separate initiative.

- HCC performance moved fast and kept moving: +8% raw improvement in the first six months, +24% at the one-year mark.
- Category-level gains included secondary hypercoagulable state (26% to 58%) and severe morbid obesity (40% to 70%). Unspecified depression coding fell from 42% to 23%, an improvement, because diagnoses moved to more specific codes.
